[Form Builder] CSS breaking for notes in table control

Description

CSS is breaking for the notes in table control in the implementer interface. A commit has been missed from the pr raised against the Jira card.

tested with a table control and placed 2 obs control inside, enabled notes .. work fine in form builder - design mode and preview mode. also works fine while capturing data in clinical consultation

As mentioned in the description, the css changes got missed as part of raising the PR for https://bahmni.atlassian.net/browse/BAH-748

The below PR just has div tags with classnames which haven’t used before. Tested the changes and don’t see any diff with applying the css changes but nothing is breaking either in form-builder or in bahmniapps side. Merged the PR to product master branch. Below are the commit details
