[Troubleshoot] concept endpoint called after hitting save takes time to respond for patient with multiple visits and encounters

Description

While performing consultation for a patient with 1000+ visits and 250+ encounters, on clicking the save button, it was observed that the following endpoint was called which took higher response time.
