Context:
In retrospective mode, users can enter the patient data at a later point in time. There are scenarios when this is done by a person (e.g. data entry clerk) who is not the provider with whom the patient had an encounter. So the retrospective mode provides the ability to mention the "Enter on behalf of" field where we capture the provider with whom the patient had the actual encounter.
The observations captured in this mode will be indicated with these provider details on the visit /patient dashboard.

Steps to Reproduce:
1. Login as Superman and access any patient's dashboard.
2. Click on the login location button on the top right and enter the provider name and a date in the past for which you want to capture the patient observations. (note: the provider must be a provider in openMRS)
3. The strip on top turns blue once you have entered the retrospective mode.
4. Enter into consultation and capture some data (fill an obs form). Save the data
5. Access the dashboard and check for the observations

Actual Behaviour
In version 0.90 the "enter on behalf of" provider is not being displayed along with observations captured for a patient. It shows the logged in user (superman) details only : super man 12.00 am

Expected Behaviour
The observations must show "superman on behalf of <Provider name> 03.29 am"
