Add dummy location in default-config to create surgical block in OT module

Description

To create a surgical block in OT module, we should have at least one location tagged as "Operation Theatre". And we don't have any location which tagged as "Operation Theatre".
As part of this card we will be adding one dummy location to default-config as a migration script.

As this migration is default-config specific, other Bahmni Implementations don't get migration with installation. So we need to document detailed steps on how to create a new location and tag it as "Operation Theatre".

Added migration script which will add default location for OT module. Below are the commit details
