If you or your organization is already planning a specific project (or has started it already) and you want it to be incorporated into the core Bahmni product, then one of your first steps should be to describe your plan on the Discussion Forums, and ask the community to comment on how this should fit in the core product. Note that no features will be added to Bahmni without going through a Public Design Process.

What we use the issue tracker for
We use the issue tracker for project management purposes, so people can be aware of the work that is ongoing, and to record what was included in different releases.
What we do not use the issue tracker for
We do not use the issue tracker for detailed design discussions, because it does not provide a good conversational user experience. These kinds of discussions should happen in the Discussion Forums. The standard process would be to create a discussion topic with a title like "Design for feature X (BAH-123)" and to link to this from the JIRA issue.
